Guidance for completing a Child Performance Licence application

The application for a Child Performance Licence is in two parts.

Follow the guidance below when completing part one and two, and the medical declaration.

Part one

Part one of the application form should be completed by the applicant.

Questions 1 and 2

You should complete all the questions in this section.

Question 3

You should provide a full description of what the child will be doing while performing. This helps in considering any specific conditions that may need to be included on the licence.

Question 4

You should ensure that the risk assessment is attached when submitting the application.

Question 5

You should provide the full address of the venue or location, including the venue name and postcode.

If the location does not have a postcode (e.g. beach or park), you should provide the postcode of the nearest building or where the unit base will be situated. 

Questions 6, 7, and 8

You should provide all dates and time of day that the child will be required to perform or rehearse. Stating 'within permitted hours' is not acceptable.

If specific dates are unknown, you should provide a start and end date (up to a maximum of six months in the future) and the number of days required within that period.

Questions 9 to 13

You should complete all the questions in this section.

If a question does not apply, you should use 'not applicable’ or N/A. You should not leave any questions unanswered.

Questions 14 and 15

You should provide the full name and address of the chaperone(s) and the local authority they are registered with.

If a parent, teacher, or tutor will accompany the child instead of a chaperone, you should provide their full name and address.

Questions 16 to 19

You should complete all the questions in this section.

If a question does not apply, you should use 'not applicable’ or N/A. You should not leave any questions unanswered.

Part two

Part two of the application form should be completed by the parent or guardian.

You should complete all the questions in this section.

Questions 5 to 9

You should complete all the questions in this section.

If a question does not apply, you should use 'not applicable’ or N/A. You should not leave any questions unanswered.

Medical declaration

The medical declaration should be completed by the parent or guardian.

You should ensure that every box on the medical declaration is completed. If a section does not apply, you should use 'not applicable’ or N/A. You should not leave any sections blank.

The parent must provide a signature and print their name in the respective boxes.

The applicant must sign and date the application form.
