Carer's assessment and support

Our carer’s assessment and support service is designed to help you think about your situation and consider what support you need in your role as a carer.

There are now different ways you can have a support conversation or complete a carer’s assessment, so you can choose what works best for you.

New: AI carer’s support service

You can use our new AI carer’s support service – a simple online tool that you can use to talk about your caring situation, at a time and place that suits you.

With the AI carer’s support service you can:

  • Have a conversation when it suits you, whether that’s during the day, late at night, or whenever you have a quiet moment
  • Get personalised support suggestions straight away with a helpful summary at the end of your chat
  • Pause and return whenever you need to – pick up right where you left off
  • Choose the pace - have a quick chat for immediate advice, or take more time to explore your situation in detail.

This is a new, flexible way to have a support conversation or complete a carer’s assessment. You’ll still have the option to speak with a member of our Adult Social Care team if you prefer.

Before you start

Think about what works well in your life, how you currently manage day to day and what you would like to change. It may be helpful to do this with a family member or someone else you trust.

You will need a valid email address if you wish to receive a summary of your conversation.

How sharing your information helps you

The information you share with us will help us to understand your caring role and the impact it has on your life. This enables us to provide advice and information that is relevant to your circumstances, connect you with local services that may be able to help and determine if you are eligible for support from the council. 

Anything you tell us will be stored securely and reviewed by experienced social workers to ensure any advice or support is fair, consistent and based on your needs. All information is handled in line with data protection law and our privacy policy

About the services we suggest to you

During your conversation, the service may suggest organisations that could help, such as charities or community groups. These links are provided to make it easier for you to find support that fits your needs.

Please note that these are suggestions only. Whilst every effort has been made to ensure the accuracy of information we provide, the council cannot guarantee the quality, reliability, or performance of all third-party services. We encourage you to explore your options and decide what feels right for you.

  • Contact Wandsworth Carers Centre to find out more about local support and activities.

Whichever option you choose, once you’ve shared your information with us a member of our team will be in touch to discuss your situation.
